GENERAL DENTAL INFORMED CONSENT

All patients must understand that the dentistry is not an exact science and that no dentist can properly guarantee the results.

All patients must authorize any of the doctors or dental auxiliaries of MagnifiDENT Inc. to proceed with and perform the dental restorations and treatments as explained to them. All treatments will be subject to modification depending on undiagnosable circumstances that may arise during the course of treatment. They must also understand that regardless of any dental insurance coverage they may have, they are still responsible for payment of dental fees, any attorney's fees, collection fee or court costs that may be incurred to satisfy any obligation to this office.

All dental treatment must be properly explained to the patient and they must give their full consent prior to any dental treatment as SOP from MagnifiDENT Inc.

TREATMENT TO BE DONE

The patient must understand and consent to have any dental treatment done by the dentist in this clinic, the risk and benefits and cost have been fully understood before proceeding to any kind of treatment.

Those treatments include but are not limited to x-rays, cleanings, periodontal treatments, tooth fillings, jacket crowns and bridges, all types of extraction and surgery, root canals treatment, dentures/prosthesis, local anesthetics, dental implant, tooth whitening, orthodontic treatment (comprehensive or adjunctive) and TMJ disorders.

DRUGS AND MEDICATION

The patient must understand that antibiotics, analgesics, and other medications can cause allergic reactions causing redness and swelling of tissues, pain itching, vomiting and/or anaphylactic shock. All drugs have side effects whether they are prescription or non-prescription.

Anything we put in our mouth can cause a negative reaction. Some medications don't mix well with others you may be taking, so once you start taking medications, you always have to be diligent to keep track of possible interactions and see your doctor immediately in case of bad reactions.

RADIOGRAPH / X-RAY

The patient must understand that x-ray shot or radiograph maybe necessary as part of diagnostic aid or tool to come up with tentative diagnosis of your dental problem and to make a good treatment plan, but this will not give the dentist a 100% assurance for the accuracy of the treatment since all dental treatments are subject to unpredictable complications that later on may lead to sudden change of treatment plan and subject to new charges.

TOOTH FILLING

The patient must understand that care must be exercised in chewing on all kinds of tooth fillings, especially during the first 24 hours to avoid breakage.

A more extensive filling or a crown maybe required as additional decay or fracture may become evident during and after initial excavation. Significant sensitivity is common, but usually a temporary effect of a newly placed tooth filling. Filling a tooth may irritate the nerve tissue creating sensitivity and treating such sensitivity could require root canal therapy or extractions as necessary.

REMOVAL OF TEETH

The patient must understand that there are alternatives to tooth removal these are root canal therapy, crowns, dental implants and periodontal surgery, etc. and the patient must completely understand these alternatives, including their risks and benefits prior to authorizing the dentist to remove their teeth and any other necessary for reasons above.

The patient must understand that removing teeth does not always remove all the infections if present and maybe necessary to have further treatment. The patient must also understand that risk involved in having teeth removed, some of which are the pain, swelling, spread of infection, dry socket, loss of feeling teeth, lips, tongue and surrounding tissue that can last for an indefinite period of time or a fractured jaw. The patient must understand that they may need further treatment by a specialist if complications arise during or following treatment, cost of which is already patient's responsibility.

DENTURES / PROSTHESIS

The patient must understand that wearing dentures can be difficult. Sore spots, altered speech, and difficulty in eating are common problems. Immediate dentures (placement of denture immediately after extractions) may be painful. Immediate dentures may require considerable adjusting and several relines. It is the patient's responsibility to return for delivery of dentures.

Failure to keep patients delivery appointment may result in poorly fitted dentures. If a remake is required due to patient delays of more than 30 days, there will be additional charges. A permanent reline will be needed later which is not included in the fee. All adjustment or alterations of any kind after this initial period are subject to charges.

ROOT CANAL TREATMENT

The patient must understand that there is no guarantee that the root canal treatment will save a tooth and that complications can occur from the treatment, and that occasionally root canal filling materials may extend through the tooth which does necessarily affect the success of the treatment.

The endodontic files and drills are very fine instruments and stresses vented in their manufacture and calcifications present in teeth can cause them to break during use. The patient must understand that referral to an endodontist for additional treatments may be necessary following any root canal treatment, and the patient is responsible for any additional cost for treatment performed by the endodontist. The patient must also understand that a tooth may require extraction in spite of all efforts to save it.

PERIODONTAL DISEASE

The patient must understand that periodontal diseases are a serious condition causing gums and bone inflammation and bleeding that can lead to the loss of your teeth. The alternative treatment plans to correct periodontal disease includes

CAPS & BRIDGES

Preparing a tooth may irritate the nerve tissue in the center of the tooth, leaving your tooth feeling sensitive to heat, cold and pressure. Treating such irritation may involve using special toothpaste and mouth rises or root canal therapy.

The patient must understand that sometimes it is not possible to much the color of the natural teeth exactly with artificial teeth and also they may be wearing temporary crowns which may come off easily and that they must be careful to ensure that they are kept on until the permanent crowns are delivered.

It is the patients' responsibility to return for the permanent cementation within 20 days from tooth preparation, as excessive delays may allow for tooth movement which may necessitate a remake of the crown, bridge or cap.

The patient must understand there will be additional charges for remakes due to the delaying permanent cementation, and they must realize the final opportunity to make changes in their new crown, bridge or cap (including shape, fit, size and color) will be before permanent cementation.

ORTHODONTIC TREATMENT

Orthodontics is a specialized branch of dentistry that diagnoses, prevents and treats dental and facial irregularities called malocclusions.

PURPOSE

Although orthodontic treatment often improves facial appearance and occasionally is performed for solely cosmetic reasons, it is used primarily to correct health problems and to ensure the proper functioning of the mouth.

Properly aligned teeth, which close together correctly, simplify oral hygiene and enable children to chew their food efficiently.

Orthodontic treatment provides the following:

  • Straightens teeth that are rotated, tilted or otherwise improperly aligned.
  • Corrects crowded or unevenly spaced teeth.
  • Corrects bite problems.
  • Aligns the upper and lower jaws.
